Case Description
My bicuspid/premolar is possibly cracked. Possbily beneath the gum line. Need root canal and crown or possibly a dental implant. What are the costs? Which procedure is best? What kind of Crown material is best?

There are many options available. Each situation must be evaluated very carefully and in a case by case evaluation. There are many different types of crown materials and all of these depend on how your bite is, what your opposing teeth look like, what type of esthetics are you looking for. Also the biggest question should be, why did this tooth break? If there is a source that has caused this tooth to break, and teeth are very strong and should not break easily unless some chronic force has weakened them and something else breaks them, and the source still exists, then whatever you do to try to place a new crown, root canal or implant, you will do the same damage to them as well, so it will be a waste of your time and money. Depending on the complexity of your case, you might be looking at fees ranging from $1200-$14000 depending on the complexity and the amount of damage. Cost Estimate: $1500 - 5000
Your tooth needs to be evaluated to see if indeed it is cracked, and the direction and depth of the fracture. This evaluation will need to include current radiographs. If the tooth is fractured under the gum line, there will be very little tooth structure to grab on to retain the crown. This would necessitate root canal therapy and a post for retention. The cost for a root canal, post and crown could range from $ 1500 to 2500. Again, depending on the amount of tooth structure left, this may be a very weak situation and have a poor long term prognosis. A dental implant might be a much stronger solution and have a much better long term prognosis. Extraction, the placement of the dental implant and the restoration could probably cost between $ 3500 to 5000.

The type of crown material is reccomended based on esthetics, strength, the opposing tooth and the bite. No one material is best for all situations.

You are in a catch 22 situation. My advice to you is to first get an accurate diagnosis for your situation. It is important to find out the source of the problem and address it so it is not a perpetual issue. I would not base my decision entirely on cost. I encourage you to make a consultation appointment and ask all the questions that you have during the visit. Your dentist must welcome that. If he/she is not willing to spend the time to educate you, then find one that will. By proper guidance and care, you will prevent many future concerns.
